Appointment Information
Appointments can be made by calling our Centralized Scheduling Department at (303) 685-2766 or 1-866-300-7326 toll free outside of Denver.
Insurance Information
The physicians of Colorado Rehabilitation and Occupational Medicine participate with most insurance companies. Your insurance may require a referral from your primary care physician. Co-payments are expected at the time of service. Please check with your insurance carrier regarding deductibles.
Cancellation / No Show Policy
If you are unable to keep your appointment, we ask that you contact us as soon possible to cancel or to re-schedule. Our policy allows us to bill you $100 for evaluations and procedures and $50 for follow-up appointments if you do not notify us at least 48 hours in advance of your appointment. The same fees apply if you do not show up for an appointment.
Patient Information
Please print the patient information form (English) or patient information form (Spanish) and the patient intake questionnaire and answer all questions as accurately and thoroughly as possible. Please bring the completed form to your appointment.
